Mental health focus: Meet care coordinators

VALLEY — Navigating the healthcare industry can be a tricky task. ICHP Care Coordinators are able to assist with any questions or concerns regarding healthcare services available to residents the San Luis Valley.

The San Luis Valley Behavioral Health Group has implemented the Integrated Community Health Partners program or ICHP. ICHP is a Medicaid program, which consists of a group of doctors and behavioral health providers that formed a team to meet the healthcare needs of those utilizing Medicaid services.

The goal of this program is to assist with healthcare needs and educate the community of the available services. The ICHP Care Coordinators help individuals understand their health problems. They work with the individual and their doctor to get the needed care. They connect individuals with programs focused on their condition, providing answers about their personal health and/ or medications. Care Coordinators help find primary care providers and ensure the individual is a part of the treatment team working towards their healthcare goals.

Care Coordinators aid with all things health related. Including those thing that could be preventing an individual from getting the care they need. There are resources available to help with transportation, financial assistance for utilities, housing, food and clothing. There are even resources to assistance with legal services, paying for prescription drugs, vision and dental care, behavioral/ mental health care, and drug and alcohol treatment.
